REQUEST FOR STATEMENTS OF QUALIFICATIONS for CONSTRUCTION MANAGER as CONSTRUCTOR where the Basis of Payment is Cost of the Work Plus a Fee without Guaranteed Maximum Price.
The Jackson Hospital Board and its governing board (collectively, “the Hospital”), located in Marianna, Florida, is accepting Statements of Qualifications from Construction Manager as Constructor (collectively, “the Contractor”) Firms under the provisions of the Consultants’ Competitive Negotiation Act, which is codified at Section 287.055, Florida Statutes.
PROJECT DESCRIPTION:
The scope of work for this project includes General Contractor services for the construction of a 1-story Physical Therapy Center at 3030 Hospital Drive, Marianna, Florida 32446. The Facility will house a physical and speech therapy area, and swimming pool with male and female locker/shower area. The Building is to be a Slab on Grade, with Spread Foundation, and Concrete Block Bearing Walls. The roof is to be Bar Joist and Metal Deck Structure with Non-Venting insulating Concrete and fully Adhered TPO roof membrane, per drawings prepared by Mason Blau and Associates. (A Stengle Hill company). The facility is approximately 10,156 square feet, non-sprinklered. Project Cost Range in Anticipated to be $3,000,000.00 to $3,600,000.00. The Hospital will pick a minimum of 3 Contractors as prequalified bidders for an interview.
INFORMATION PROVIDED:
Firms interested in being considered as candidates are required to submit five (5) bound statements of qualifications, to be organized in the following order and including, at a minimum, the following information:
1.A copy of the firm’s current applicable Florida Construction license(s) and corporate registration certificates.
2.Complete AIA document A305, Contractor’s Qualification Statement, latest edition.
3.Proof of general, automobile and workers’ compensation liability insurance coverage.
4.A complete list of the firm’s relevant insurance policies, including coverage limits, demonstrating that the firm maintains minimum insurance coverage sufficient to protect the Hospital for this project. 5.Proof of current bonding capacity, including total bonding capacity, current usage, and available bonding capacity remaining.
6.A statement as to whether the firm is a certified Minority Business Enterprise (MBE) or Woman or Minority Owned Business Enterprise (WMBE). If your firm claims MBE/WMBE status, a copy of your Firm’s current, valid MBE/WMBE certificate is required as part of the submission package.
7.Contractor is familiar with the Davis-Bacon Act and projects. 8.Resume of key personnel assigned to this project. Include individual relevant construction experience as follows:
a.Construction of projects similar in size and scope as described above in an active, hospital building campus.
b.Construction of hospital projects similar in size and scope as described above in an active, hospital campus, preferably in the state of Florida.
c.Examples of a similarly completed construction project that include: i.Construction on an acute care hospital campus.
ii.Construction of indoor swimming pools of similar size and occupancy type.
iii.Experience with the Davis-Bacon Act.
9.Firm’s construction experience including, but not limited to: Roadways, Surface Parking, Utilities, Lighting, Landscaping, and Drainage
10.A list of at least five client references consisting of name, title, organization, address, telephone number and project name(s) for each reference specified. Please ensure your references are updated and willing to reply. Reference responses are mandatory.
11.A summary of construction permitting experience within the City
of Marianna, Florida, and other applicable Florida Authority having jurisdiction.
12.The location of the contractor's main office, and proposed project team office location (if different from main).
13.An explanation of how the firm will respond expeditiously to urgent project matters, including a summary of the firm’s proposed approach to this particular project.
The basis for selection criteria for this project includes, but is not limited to:
1.The Contractor’s relevant experience, as it relates to the above- referenced project.
2.The Contractor’s relevant experience with regulatory agencies with applicable jurisdictional authority, including, but not limited to, federal, state, and local agencies.
3.The Contractor’s dept of construction management team and the resources available for this assignment. This includes recent, current, and projected workloads of the firm.
4.The location of the Professional General Contracting Firm’s main/ corporate office.
5.The Contractor’s approach to this particular project.
6.The Contractor’s ability to respond quickly.
7.Whether or not the lead Firm is an MBE/WMBE (valid certificate from the Office of Supplier Diversity required).
8.The volume of previously awarded projects to the Contractor by the Hospital.
9.The Firm’s available bonding capacity for this project.
10.Experience with the Davis-Bacon Act.
The Selection Committee may use a scoring rubric or consensus method to evaluate qualifications based on the criteria described above. No price or fee proposals shall be considered during the evaluation phase, in accordance with Section 287.055, Florida Statutes. It is the Hospital’s responsibility to negotiate a fair, competitive and reasonable compensation per Section 287.055, Florida Statutes. A fair, competitive and reasonable cost of work plus fee shall be evaluated based upon the following information: (1) Other similar projects; (2) other cost reference data; and (3) after approval of the ranking, bids from the shortlisted Contractor firms based upon the drawings and specifications as prepared by the hospital design team shall be negotiated.
All interested Contractors are further informed as follows:
1. The Hospital reserves the right to reject any or all submittals at any time during this process.
2. The Hospital reserves the right to request additional information beyond the data set forth above.
3. The Hospital reserves the right to waive any minor irregularities, informalities, or technical deficiencies in a submittal, provided that such waiver does not materially affect the qualifications of the responding firm.
4. Questions regarding submissions shall be directed only to Rex Torbett, rtorbett@jackhosp.org. Submissions shall be titled: Statement of Qualifications for Construction Manager as Constructor Services, Jackson Hospital, Jackson Physical Therapy Center, Marianna, Florida. 5.Submittals must be received by the Hospital no later than 2:00 p.m. on Monday, August 25, 2025. Submit statements to Rex Torbett, Jackson Hospital, 4250 Hospital Drive, Marianna, FL 32446. Submittals received after this deadline will not be accepted. Facsimile (FAX) or electronic submittals are not acceptable and will not be considered. Applications that are late or do not comply with the above instructions may be deemed non-responsive and rejected at the sole discretion of the Hospital. Submittals are part of the public record. Application materials will not be returned.
6. Only Rex Torbett shall be contacted with regard to this Request for project-related questions. Requests for in-person meetings by individual firms will not be granted. No communication shall take place between the applicants and the Hospital’s Selection Committee members or employees of the Hospital. Failure to comply could result in immediate disqualification at the discretion of the Hospital.
7. The selection committee will meet in a public meeting located at Jackson Hospital, 4250 Hospital Drive, Marianna, FL 32446,on the ground floor board room to discuss and announce the top three ranked Firms (unless fewer than three Firms deliver submittals) deemed to be the most highly qualified to perform the required services with whom the Hospital will subsequently engage in contract negotiations. If there are less than three (3) Firms responding, the Selection Committee shall also evaluate current statements of qualifications and performance data on file with Hospital, in determining which Firm is the most highly qualified to perform the required services and announce the top three ranked Firms.
